My goal as a therapist is to work with my clients to help them use their natural strengths and abilities to move past their challenges. I believe that most people already possess the natural abilities and talents to move past mental health or sleep challenges, but might need some coaching or occasional suggestions in order to do so. I utilize therapeutic methods that have been rigorously researched and studied. I have extensive experience working with adults who are experiencing anxiety, depression, insomnia, and trauma.

Growing up can be challenging for adolescents- both in regards to mental health and in their relationships with peers. While mental health has become more prominent, there is often a hesitation that arises when it comes to discussing it. I offer a space, separate from their daily life and relationships, where adolescents can speak and feel heard by someone uninvolved and unbiased, but invested in being present and supportive of the adolescent. Adolescents are facing new obstacles both socially and within themselves, and while opening up can be hard, it is so important to do.

Welcome! It can be hard to recognize that you might have parts of your life that you have pushed aside or are uncomfortable with. It takes a lot of courage to acknowledge this and then to take the steps to reach out to someone for assistance. I believe in working as a partner with people to explore areas that cause difficulty, to build upon solutions that have previously worked, and to learn new skills to improve one's ability to cope. I enjoy getting to know the people I work with and finding solutions that are unique to them. One of my greatest professional joys is being with someone as they grow in ways that allow them to find calm, confidence, and better ways of coping in this wild world. If you feel we might be a good match I look forward to hearing from and getting to know you. Telehealth provided across NY State only. In office appointments in Croton-on-Hudson, NY. I am accepting new clients and have availability in the morning hours (9am-12pm).

As a Licensed Clinical Social Worker with over a decade of experience, I specialize in ADHD, autism spectrum disorder, anxiety, and perfectionism. My clients say that they find my honest, warm, and straightforward approach to be refreshing and helpful. Whether you're dealing with relationship issues, school stress, or a sense of being stuck, these obstacles can trigger anxiety, depression, and other mental health concerns. High standards, whether self-imposed or external, might feel impossible to meet, leaving you feeling overwhelmed. You don't have to face these challenges alone. Together, we can develop effective strategies to help you embrace life fully, fostering a sense of control and freedom. I specialize in supporting adults and teens, particularly those who identify as LGBTQIA+ or performing artists, who are feeling anxious, sad, and overwhelmed. My approach helps manage anxiety, depression, ADHD, insomnia, and OCD, offering thoughtful conversations and practical tools to get you back to living fully. Finding the right therapist can be daunting, but I'm here to make the process easier. Call me Michael, not "Doctor Jacobs." I consider my approach to be direct, warm and less formal. I am a Psychiatric Mental Health Nurse Practitioner (PMHNP) specializing in the evaluation and treatment of anxiety disorders, depression, bipolar disorder, ADHD, trauma-related disorders, obsessive-compulsive disorder, insomnia, and other common mental health concerns for patients across the lifespan. Building trusting, collaborative relationships is central to my practice, as I believe the best outcomes come from working together to create treatment plans that align with each person's goals. Patients are encouraged to be active advocates for their own mental health, while also knowing they have a dedicated partner supporting them in achieving recovery, maintaining stability, and improving their overall well-being. Everyone deserves compassionate, evidence-based mental health care personalized to their unique needs and goals. My role is to be a supportive, collaborative partner who will listen without judgment, celebrate your progress, and help you navigate challenges along the way. Whether you're seeking help for the first time, managing an acute concern, or working to maintain the stability you've achieved, we'll create a plan together that fits your life and empowers you to take an active role in your care. I look forward to helping you build resilience, strengthen your confidence, and work toward lasting mental wellness one day at a time, bringing empathy, collaboration, and a whole-person perspective to every visit so you feel supported, understood, and empowered. Mental health care, in my view, is not only about treating symptoms but also about developing the skills and support needed to manage daily stressors, which are a natural part of life. This means helping patients feel prepared and empowered to navigate challenges without letting them become overwhelming or destabilizing. I am passionate about providing comprehensive care that meets people where they are, whether that includes medication management, psychotherapy, or an integrated combination of both to support long-term mental health and well-being. My approach to care spans the full spectrum of psychiatric acuity, from navigating more acute mental health challenges to maintaining stability and preventing relapse after achieving wellness. Understanding each person's unique experiences, goals, and needs comes first, which I achieve by bringing a compassionate, and individualized approach to every relationship. You can expect a kind, supportive, and collaborative environment where you feel heard, respected, and empowered as we work together. This approach is also encouraging, offering gentle guidance and honest feedback when needed to support meaningful growth. Lasting change happens through partnership, and you are encouraged to take an active role in your own mental health journey. I understand that making these changes can feel challenging, so my goal is to meet you where you are while helping you build confidence, develop effective coping strategies, and move toward greater stability and independence. Every treatment plan is tailored to your unique needs, experiences, and goals, so it feels both realistic and achievable. I began my career in a hospital-based outpatient clinic, where I worked with individuals from diverse cultural backgrounds and varying levels of psychiatric acuity, along with six-and-a-half years as a registered nurse in pediatric critical care and pediatric neurology/neurosurgery. Caring for children and supporting families through some of life's most difficult moments showed me how deeply mental and physical health are connected, and how essential accessible, competent, individualized, and compassionate mental health care is to overall well-being. These experiences inspired me to pursue psychiatry and continue caring for people in a different way. Today, I have the privilege of working with patients across the lifespan, and I truly enjoy helping individuals at every stage of life navigate unique challenges. I love cooking, hanging with family and friends, and spending time in the garden. I'm also a devoted New York Rangers hockey fan!

Students who come to work with me often share some things in common: -They're high-achievers. Driven, capable, and holding it together on the outside, but quietly exhausted, burned out, or feeling like something's missing on the inside. -They might be navigating anxiety or OCD, going through a major life transition, or questioning their purpose and what comes next. They may be working through shifts in their relationships, with a partner, with their parents, or with themselves. -Many have never asked for help before. They're used to being the one who has it together, and reaching out takes real courage for them. If any of that sounds familiar, reach out for a free 20-minute consultation and let's get started together :)

I believe the most important foundation in therapy is the relationship between therapist and client. The therapy relationship can be a safe place to practice new skills that you are hoping to bring back to the rest of your life. My goal is to help you to feel safe during your time with me, so that you can explore yourself, your challenges, your strengths, your history, and anything else that is important to you. I also hope to help you make the changes that you would like to make in your life. Maybe you can feel better than you do now. Maybe you can learn more skills to cope with what feels hard. Maybe you need to be heard and validated. Whatever brings you to therapy, I’m here for it.

In my years of experience I have come to realize so much about living in the world robs us of our ability to acknowledge our own magic. Therapy should be a safe place for you to begin to reclaim that magic and learn how to discover your own power. By using a "bottom to top" approach to identify how long held, negative beliefs, have kept you reliving and repeating the past, you can finally live in the present. I provide a safe, non judgemental space for you to work through trauma and reclaim the power inherent in you.

I work with individuals across the adult lifespan. I find great joy serving college students and graduate students, and professionals in academia. I have spent many years working in university counseling centers including University of Iowa, UC Davis, CSUN, Indiana University and Cornell Health. My practice tends to focus on helping clients improve their relationship with self, others and the world around them. Some common themes in my practice are self-discovery; self-compassion; gratitude; boundary setting; clarifying values, needs and wants, and intention; mindset; health and wellness; mindfulness; and finding ways to live more present and fully.

I’m glad you’re here. Looking for support is a big first step. Therapy can be a place to slow down, to make sense of what you’re dealing with, and find new ways forward that feel more manageable. I have extensive experience working with college-aged and emerging adults navigating identity, relationships, and life transitions. Additionally, I've worked with adults on the autism spectrum, family members and caregivers of people with developmental disabilities, and others in helping roles. More importantly, I’m interested in getting to know you as an individual and working together in a way that feels supportive, practical, and aligned with who you are. Therapy works best when it feels like a partnership. I’ll listen, guide, and support you as you build on your strengths, tackle challenges, and develop practical tools for growth. Together, we’ll create a safe space where you can feel understood, empowered, and more confident in yourself.

My top priority is to provide you with a safe, compassionate, and nonjudgmental space where we can explore your thoughts, emotions, and experiences. We will work together towards developing insight, fostering self-compassion, and finding strategies to help you live a life that feels authentic and fulfilling. We will set goals together, tailoring our approach to suit your individual needs and preferences. I have had the honor to work in the field of mental health for the past 15 years and journey alongside amazing people who were able to discover the strength, grace, and wisdom waiting for them. While I have experience providing several treatment modalities, I specialize in Mindfulness-Based Stress Reduction (MBSR), Narrative Therapy, and Cognitive Behavioral Therapy (CBT). I have also been extensively trained in Trauma-Focused Cognitive Behavioral Therapy (TF-CBT). Additionally, for the past eight years, I have taught courses on human behavior and additional topics at SUNY Brockport and Nazareth College allowing me to stay current on mental health research and treatments. My experience has shown me that everyone has the capacity for growth, we sometimes just need a space to discover and map out the life that we want. I would be honored to provide that space and help you bring more authenticity into your life.
